Make a Family Support Referral

Please note we do not accept self referrals and referrals must be made by a professional.


To make a referral you will need to have a Referral Partner ID and an Organisation ID.

These will have been emailed to you when you registered.

 

What is a referral?

​

A referral at Sebby's Corner is the process by which families are referred to us by a professional, such as midwives, health visitors and teachers. These families have typically been flagged as needing aid to get them out of adverse living situations. It could be those living in poverty, temporary accommodation, women and their children who have fled domestic abuse, refugees, asylum seekers and victims of modern slavery and human trafficking.


A collaborative approach ensures that families receive comprehensive care, addressing their unique circumstances and aiding their overall well-being. Our referral system plays a crucial role in creating a cohesive safety net for vulnerable families.
